GENERAL AFFIDAVIT
State of Ohio County of Greene SS.
In the matter of Ary Smith widow of Jack Smith "C" 100 U.S.C.T. #261732 ON THIS 6th day of August A.D. 1883 personally appeared before me a Clerk of Court in and for the aforesaid County, duly authorized to administer oaths Matthew Pendleton aged 37 years, a resident of Xenia in the County of Greene and State of Ohio whose Post Office address is Xenia Ohio and well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows: I was a private in Company "C" 100 U.S.C.T. and was well acquainted with Jackson Smith of same company & regiment I was acquainted with him for about four years before he enlisted was very intimate with him he was a stout & healthy man at his enlistment I remember that while our regiment lay at Nashville at Camp Foster said Smith took sick with heavy cold and was sent to hospital at Nashville Tenn was there about four months afterwards he joined our regiment Then he was sent to Hospital again for a wound he [] at Nashville Tenn twice we fought [] & [] he was in hospital there about 3 months then he was sent to hospital last time while we were at Stevenson Alabama he followed our regiment from Nashville after the fight. He was sent from Stevenson Ala to Nashville Tenn. I cant give these dates but I remember well the circumstance and that he had a very bad cold when he was sent to Hospital first time and had a bad cough I knew him well from his discharge until his death and know he never recovered his health That he always complained of lung trouble after the war, had a bad cough and died of that disease. We both lived here in Xenia from his discharge untl his death in 1874.
Affiant further declare that he has no interest in said case and is not concerned in its prosecution

Sir:
I have the honor to return herewith your request for a report of hospital treatment in Claim, No. 361432, with such information as is furnished by the records filed in this Office, vis: that Jack Smith Private Co C 100 U.S.C.T. was admitted to No. 16 G. H. Nashville Tenn Jan 5, 1865 with Pneumonia and returned to duty Jan: 16, 1865. No further record of Soldiers treatment found. There are no records of the Regiment on file, nor of Decater Ala subsequent to Nov 1864.

I have the honor to acknowledge the receipt of your request of July 20th, 1881, for certain information for use in the consideration of application for Pension No. 261,732 and to return it herewith, with the following information from the records of this office:
It appears from the Rolls of U.S. Colored Troops on file in this office that Jack Smth was enrolled in Co. "C" 100th Regiment of U.S. Colored Troops
The records also furnished the following information in his case: Born in Clark Co Ky: age at enlistment, 30 years; occupation a, Laborer; eyes, black; hair black; complexion, black; height, 5 feet 7 inches. He was a slave of Daniel Smith.

SIR:
I have the honor to acknowledge the receipt of your request of June 8th, 1880, for certain information for use in the consideration of application for Pension No. 261,732 and to return it herewith, with the following information from the records of this Office: It appears from the Rolls of U. S. Colored Troops on file in this Office that Jack Smith was enrolled on the 14" day of May 1864 at Lexington, Ky in Co. "C" 100th Regiment of U.S. Colored Troops, to serve 3 years, or during the war, and mustered into service as a private on the 14" day of May 1864, at Lexington Ky, in Co. "C" 100th Regiment of U.S. Colored Troops, to serve 3 years, or during the war. On the Muster Roll of Co. "C" of that Regiment from Orgn: to Oct 31", 1864, he is reported present for duty; Nov & Dec" A. S. at Decatur Ala. since Dec 27"; Jany & Feby "65, absent sick Nashville Tenn since Feby of 65"; from Feby 28/65 to Oct 31/65 present for duty.
He was mustered out of service with company December 26th 1865, at Nashville Tenn.
