PHYSICIAN'S AFFIDAVIT
State of Ohio County of Greene
of Ary Smith widow of Jackson Smith late of "C" 100 U.S.C.T. # 261732
Personally came before me, a Clerk of Court in and for the aforesaid County and State Dr. C. B. Jones a citizen of Xenia O whose Post Office address is Xenia Greene Co Ohio well known to me to be reputable and entitled to credit, and who, being duly sworn declares in relation to aforesaid case as follows:
That he is a Practising Physician, and that he has been acquainted with said soldier for about 10 years,
I was called to attend Jack Smith Sept 7th 1873 and saw him frequently btwin that date and March 19th 1874 when my visits to him ceased. He was suffering from Consumption. I have no previous accquaintance with him.

GENERAL AFFIDAVIT
State of Ohio County of Greene, SS:
In the matter of pension of Ary Smith widow of Jack Smith late of "C" 100 U.S.C.T. # 261732
ON THIS 1st day of Febry. A.D. 1887 ; personally appeared before me Clerk of Court in and for the aforesaid County duly authorized to administer oaths, James Glass aged 45 years, a resident of Xenia in the County of Greene and State of Ohio whose Post Office address is Xenia Ohio well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows:
I first became acquainted with said Jack Smith about the time he enlisted, I was in Co. B. 100 U.S.C.T. at that time said Smith was in good health. he and I were at hospital at Nashville Tenn at same time. Smith was sick with heavy cold & bad cough he was wounded in one foot at battle at Nashville [time Gen Hosal] [was about Nashville] After the war I came here to Xenia Ohio found Smith here and he and I lived here in Xenia Ohio until he died he died here in Xenia O on the day of 18 We worked here together for several years and I knew him intimately until his death saw him daily during the time to his death and I knew he had lung disease all the time from his discharge until his death, Had a bad cough soreness in his breast [] and run down in flesh I came to Xenia Ohio in fall of 1868 at that time the Smith was here. I saw him daily from that time until soldier died and worked a great deal with him and had a a good opportunity of seeing and knowing his condition, at the time I came here in 1868. Smith told me he had been in Xenia O ever since his discharge and that the disease he had then disease of lungs cough &c was what he contracted in service. I knew that in 1871 said Smith went to Ky and married Ary Smith and brought her here to Xenia Ohio and he and Ary Smith lived here in Xenia O, as man and wife from 1871 until his death. they lived near me here in Xenia O. Said Smith never married any one else or lived with any one else as his wife but Ary Smith from the time I knew him here in Xenia in 1868 to 1871 said soldier did not have any wife and did not live with any one as his wife in Xenia O. Said Ary Smith has not remarried since the death of said Jack Smith. I further declare that I have no interest in said case and I am not concerned in its persecution. I am not related to her

ON THIS 1 day of March A.D. 1887 personally appeared before me Clerk of Court in and for the aforesaid County duly authorized to administer oaths, Richard Lamb aged 45 years, a resident of Xenia in the County of Greene and State of Ohio whose Post Office address is Xenia Ohio well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to aforesaid case as follows: I was a member of Co "K" 100 U.S.C.T. and was well acquainted with said Jack Smith knew him before he enlisted he was in good health at his enlistment. I knew he was sent to hospital at Camp Foster Nashville Tenn He had lung disease there had a bad cought very bad cought complaned of sorness of breast, I knew said Jack Smith came to Xenia soon after his discharge he and I came to Xenia O, about the same time said Smith and I lived together in Xenia O until his death. I worked with said Smith a great deal after the war until his death and know he was in very poor health from his discharge until his death and that he suffered a great deal on account of lung disease. I have seen him frequently while at work have to rest on account of his lung disease and saw him spit-up blood while out at work complained at the time of his lungs and soreness in breast and of a bad cough I knew his wife Ary Smith before the war and I knew Jack and her were married before the war that in 1871 Jack Smith and her were again married in Ky. he and I was working together at the time he went back to Ky and got married in 1871. he brought her to Xenia O where they lived as man and wife until his death. Said Smith and Ary were never married to any other person. Said Ary Smith is now the widow of Jack Smith she has not remarried since his death Jack Smith Ary his wife and I was all raised near the other in Ky.
I further declare that I have no interest in said case and I was not concerned in its prosecution
