INSTRUCTIONS FOR CLAIMANTS UNDER POLICIES.

The Documents necessary to establish the title of the Claimants-such as the Probabte of Will,
Letters of Administration, Deed or Deeds of Assignment, &c. - must be transmitted to the Office at least
fourteen days before the presentation of the Policy for payment. And where a Bonus has been added to the
Policy, such Documents must be acconpanied by the Bonus Certificate.

If an Assignment relate only to the Policy under which a claim is made, it will be retained by the
Office, but if it relate also to other property, an abstract of that part of it which has reference to the
Policy in question, together with the original Deed, must be lodged at the Office for examination; the
former to be retained, and the latter to be returned on the Assignee giving an undertaking to produce it
whenever required.

If a Policy shall have been assigned by way of security on Mortgage, it will be requisite,
except under special circumstances, on payment of the claim, that the signature of the Assignor, or his
representatvies, should be had to the receipt, in addition to that of the Assignee.

In cases of Bankruptcy and Insolvency, office copies of the appointment of Assignes must be
produced.

The following is the form of receipt for the discharge of the claim, which must be endorsed on the
Policy:-

Received this ____ day of ___ 188 __, of the Directors of the
COMMERCIAL UNION ASSURANCE COMPANY, by payment to* ____
_____the sum of ______ in full satisfaction
and discharge of all claims, including Bonuses, under the within Policy, No.____
on the life of ________________deceased.
