Squadron, Mildenhall, Suffolk. With this squadron, as air gunner
he took part in 6 operational flights, the targets including Hamburg
and Kiel, in Germany, as well as 2 sea mining operations in enemy
occupied territory.

On the 15th October, 1942, Sgt. O'Sullivan was the
air gunner of a Stirling aircraft which took off on air operations
to attack Cologne, Germany, and failed to return to its base. All
the members of the crew, including Sgt. O'Sullivan, were reported
missing. Information received through the International Red Cross
Committee revealed that Sgt. O'Sullivan had lost his life. In
consequence he was reclassified to missing, believed killed. In
due course his death was officially presumed to have occured on the
15th October, 1942. Later information received through the
I.R.C.C. that he was buried in the Cemetary of Rest, Utrecht, Holland.

There were two other R.N.Z.A.F. members in the crew-
Sgt. J.C. Brocket of Paeroa and Sgt. F.C. Pike of Christchurch.
