This form seems to have two issues:
1) The Year of Birth is wrong.
2) MOS "Construction Works" has the wrong number.
The Year of Separation is also omitted, but I'm not sure if that's an error or not. Since a year has to be input, I put 1945.
1)Per Draft Card for Bernard Taylor Beach his DOB is May 12, 1915.
2) MOS - 188 Duty Soldier II is Generic. 188 Construction Works isn't incorrect, just more specific. LVA will note and add it to list if necessary.
The years aren't required. The month and date can be entered by themselves. It is best to not enter a year if you don't have it. Per the Veterans Affairs BIRLS Death files, his Separation date was 2/15/1947.
